Low-grade MEC ( A ) displays minimal FAP-α expression in glandular stroma, while intermediate-grade ( B ) and high-grade ( C ) MEC exhibit FAP-α expression in stromal cells surrounding glandular structures. High-grade MEC ( C ) shows significantly more extensive staining, indicating higher FAP-α expression. Correlation analysis revealed strong positive relationships between ⁶⁸Ga-FAPI PET/CT imaging parameters and FAP-α expression: SUVmax correlated with staining intensity ( r = 0.91, p < 0.01) and area fraction ( r = 0.96, p < 0.01), and TBR also showed strong correlations ( r = 0.91 and r = 0.94, p < 0.01). These findings suggest ⁶⁸Ga-FAPI PET/CT parameters effectively reflect FAP-α expression, supporting its utility for non-invasive tumor biology and grading assessment.
